why is it always hard?

Well, if it's up for more than four continuous hours (or is it six?) then that is unhealthy and requires medical attention. Otherwise it is perfectly normal and healthy. It sometimes happens for no reason (in our sleep or at work and school, for example), so you could be particularly arousing to him. It's not painful unless there is something wrong.
